Plaque build up coats the inside of the arteries much like gunk in a pipe. If the pipe gets totally shut off, the heart muscle supplied by the artery is starved for oxygen. If it is very starved for oxygen, it may die. Or it may merely be starved for oxygen, but not die. The lack of oxygen makes heart muscle pump very poorly, sometimes so poorly that someone can die. Or the lack of oxygen can cause arrhythmias, where the heart can also beat irregularly and blood isn’t pumped effectively. Death can ensue in this case as well.

So plaque buildup can cause weakness of heart muscles, or death of heart muscles, or arrhythmias. All of which can make your heart pump poorly and cause death.

In less severe cases, patients may simply have chest pain, or may have shortness of breath or fatigue. It depends on how severe their heart disease is.
